We formally recommend that patients first contact their insurance provider to see if optical services are covered and to ensure Lindsey Optical is listed on their HMO/PPO.
When attending your appointment, please bring your insurance card and any necessary paperwork that your insurance may require. If you do not have any of your insurance information at your appointment time, please make arrangements to pay in full and we will provide you with any statements for your insurance required for you to be reimbursed. Since insurance plans differ, we can only quote you an estimate of what your insurance may cover but we cannot guarantee any coverage.
Payments, including co-pays, are expected at the time services are rendered. If you are unable to make a payment, please make arrangements with our Insurance Billing Department prior to your appointment.
